Social Security Form SS-5

My family member's death is listed in the SSDI as having died in Virginia in 1971, but doesn't mention WHERE in Virginia. I'm thinking of spending $27 for his SS-5, but don't know if this document will show the address or zipcode of last benefit. Has anyone ever requested a SS-5?
If so, what information did it actually show? Finding his place of death in Virginia has been fruitless to date.
