Note: I certify that the above company was mustered into the service of the State of Tennessee 25th of June, 1861 at Mossy Creek TN by Wm. A. Branner, Aid to Gen’l Caswell and served until the 4th day of Oct. 1861 when the company was regularly inspected and musted into service of the Confederate States at Camp Buckner KY. F.M. Walker Mustering Officer.

This company subsequently became Company E, 4th Branners Battalion Tn Calvary. This battalion was formed Aug. 29,1861 with six companiers of twenve mths men which had been organized in June or July 1861, for the Provisional Army of TN and afterward transferred to the Confederate States service. The Battn was re organized May 24, 1862 and then consolidated with the 5th Batt’n TN. Calvary to form the 2 ((Ashby’s) Reg. TN Calvary. It was also called the 2nd Batt’n TN Calvary.

Donelson Tn on Feb. 16th 1862 and was sent to Camp Morton Ind. Said Helton on the 15th day of Aug 1862 taken the Oath of Allegiance to the Federal Gov. and went home. Said Helton on the 16th day of Nov. 1862 rejoined his command and on the same day enlisted for the war and was taken sick at this point and died on the 17th day of Dec. 1862. David Helton private of Capt. F.M. Jackson Company G of the 81st Regt of Confederate States Army was enlisted by F. M. Jackson of the 81st Reg of TN Vols at Morristown Tennessee on the 1st day of Dec 1862 to serve three years. He was born in Grainger Co. in the state of TN He is 35 years of age. 5’7” tall. Fair complexion, blue eyes and light hair and by occupation when enlisted a farmer. During the last two mths said soldier has been unfit for duty 60 days. Said soldier is languishing under same disease of the chest and has done no duty since enlistment and has been on the sick list every since enlistment. I respectfully urge a discharge for said Soldier.

I certify that I have carefully examined the said David Helton a private of Frank M. Jackson’s company and find him incapable of performing the duties of a soldier because of the hypotrophy of the heart, dropsical effusion within the pericardium. His breathing is very short and difficult. When he walks a short distance he is overcome with palpitation of the heart. He suffers greatly. He is unfit for Jail, Hospital, or garrison duty. J.C. Grumby Company Surgeon.
