Illinois marriage questionaires of the 1870's

I ordered & received from St. Clair County IL a copy of the marriage license for my Great Grand Uncle. Along with the license was a form that he completed to get the license. It was from the "Illinois State Board of Health" and give some good information about him and his parents. The license & form were dated 31 Jan 1878.

I also ordered and received from Shelby County IL a copy of the marriage license for my Great Grandfather. It is dated 29 March 1877, but there was not a form from the "Illinois State Board fo Health". Does anyone know if all counties of Illinois required these? I would love to know if I can get one for my Great Grandfather's marriage.
