The cost of these DNA projects depends on the company. There are many companies that are doing this. Some are about $200-$300, depending on the number of markers, and some are free. The Smolenyak Company is the one I have used. I had my brother take the mouth rinse because it has to be a straight male line. There is a mother to daughter testing, going through the female line, but it's more complicated, therefore more expensive.
You don't eat or drink for half an hour. Then, using a vial with liquid that is sent you, you swish the liquid around in your mouth for a minute, then spit it in back in the little vial, then send it back. Other companies use Q-tips which you wipe inside your mouth to get the saliva on them, and send them back.
The results on the paper that is sent you, or that you can check online, is just black vertical lines under each column that is in your DNA.
So I don't know of any way a medical company, or any other company can use it. I'm pretty sure that most of the companies that do this is above board, so you can feel safe doing this.
